世預歐附加賽共12隊參加 ，他們分別是來自10個世預歐預選賽的小組第二 ，以及2個通過歐國聯獲得參賽名額的隊伍 。12支球隊分為6支種子隊和6支非種子隊 ，將分成3組 ，4隊一組，抽簽後兩兩捉對廝殺，在半決賽取勝的球隊進入到附加賽決賽 ，通過單場淘汰製的形式每組決出1個參加世界杯的名額 。其中，在世預歐小組賽中成績較好的6支球隊為種子隊 ，葡萄牙 、意大利 、俄羅斯 、蘇格蘭、瑞典、威爾士為種子隊 ，他們將在半決賽享有主場優勢 ，而決賽的對決場地將通過抽簽決定 。
北京時間11月27日0點，世預歐附加賽的抽簽將在蘇黎世進行 ，而其比賽時間則為2022年3月24日至29日。
